APPLYING FOR A PASSPORT?

Bring the following :

  • PROOF OF U.S. CITIZENSHIP (original non abstract certificate of live birth, or old or current passport, or naturalization certificate)

  • AGES 18 AND OVER NEED PROOF OF IDENTITY (valid driver's license, valid state ID, valid passport, or Citizenship Certificate)

  • AGES 17 AND UNDER NEED PARENTS PROOF OF IDENTITY (valid driver's license, valid state ID, valid passport, or Citizenship Certificate)

  • Ages 18 and over: Photocopy of Identification, front and back.

  • Ages 16-17: Photocopy of one of the parents Identification, front and back.

  • Ages 15 and under: Photocopy of both parents Identification, front and back.

  • TWO IDENTICAL PASSPORT COLOR PHOTOGRAPHS (2 in. x 2 in. with a white background)

  • Placentia Library provides passport photo servce for $12 a person (includes two photos.)

  • 1 PAYMENT TO: U.S. Department of State (check or money order only)

  • 1 PAYMENT TO: Placentia Library District (check, credit card, or money order only)

  • DS-11 Form – completed but not signed (you can also download forms at www.travel.state.gov)

  • Children under 16: Both parents must be present with child or submit a notarized statement of consent for parent unable to appear. Parents must also have photo I.D. An original birth certificate is needed for new or renewed passports.

  • Children ages 16-17: At least one parent must be present with applicant. Parent must have valid photo I.D.

  • Note: All applicants must be present.

**Passport card is good for travel from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry only.

(Allow 4-6 weeks for the return of the passport for routine service)

EMERGENCY PASSPORTS Travelers departing within 2 weeks must make an appointment with the Los Angeles Passport Agency at 1-877-487-2778 (24-hour appointment scheduling system)

PASSPORT RENEWALS: Most renewals can be done by mail–see form guidelines. Renewals less than 5-years expired are done by mail. DS-82 renewal forms are available at the Passport Office. You can also download forms at www.travel.state.gov

EXPEDITED SERVICE: If travel is in less than 3-weeks: $60.00 extra Passport Services Fee per application. Delivery in 2-3 weeks. PLUS (If Desired) Express mail postal service:$17.50 will deliver your passport via express mail rather than priority mail.
